I understand that payment is required up front before being issued a card.

I understand that a card will not knowingly be issued to CDL drivers or Federal Employees due to federal restrictions.

I understand that a CCW (Concealed Firearm Permit) may be at risk if I obtain a medical cannabis card.

I understand that cannabis has been federally classified as a schedule 1 drug and that a cannabis card may not offer protection against federal laws/regulations.

I understand that the FDA (Food & Drug Administration) has not yet approved cannabis for the treatment of conditions or disease.

I agree to contact Cache Ketamine & Mental Health in the case that I experience any concerning effects related to cannabis.

I certify that all knowledge shared with Cache Ketamine & Mental Health and my provider is truthful and accurate.
